What You'll Need to Get Started

Please have the following items ready to complete your Urgently onboarding.

Company Information — Legal business name, business address, EIN / Tax ID, local or state business registration, and primary contact information

W9 Form — Must match your business name, address, and EIN exactly

Branding Photos — Driver side, passenger side, and rear of all branded trucks; shop photos if you operate a physical location. Branding must match your business name and documents

USDOT Number — Required for operators subject to DOT regulations

Certificates of Insurance (COI) — Auto Liability, General Liability, Workers' Compensation (if applicable); On-Hook, Garage Keepers, and Cargo for towing providers. Must be active, verifiable, and list Urgent.ly, Inc. as additional insured

Your insurance agent will issue a Certificate of Insurance (COI) on your behalf. The sample below shows exactly how each coverage line must appear and who must be listed as the additional insured.
